At Drummoyne Oval, behind Maddinson's 87, the visitors posted 199 in their first innings.
In response, NSW was rolled for just 151, despite a gutsy 25 off 55 balls from Gilkes.
Scott Boland dominated for Victoria, taking 4-34 from his 19 overs.
Having already secured first-innings points, Victoria returned to the crease for their second stint.
Marcus Harris (137) and Peter Hanscomb (115) both scored centuries, with Maddison finishing two not out before Victoria declared at 3-290.
With Maddison's side chasing the outright win, the Blues went into bat for the second time.
Openers Gilkes (20) and Daniel Hughes (18) started strongly but after they were both dismissed, Peter Nevill (29 not out) was the only other batsmen to offer any real resistance - as the hosts were all out for 134, handing Victoria the outright win by 204 runs.
Boland again starred with the red ball, taking 3-39, with Jon Holland (3-16) and Will Sutherland (3-31) also grabbing three scalps for the victors - as Bomaderry High School alumnus Maddinson was named player of the match.
The two sides will lock horns in another Shield clash at the MCG from Friday, November 5 - before a One-Day Cup fixture on November 12.
