Nowra brothers Alex and Callum Mackay, Culburra Beach's Sam Wright-Smith and St Georges Basin's Tom Dolby all played pivotal roles during the three-day tournament at the Moorebank Hockey Centre.
The quartet's side, which also featured former Burrawang talent Jack Donovan, opened the tournament with two wins on Saturday, firstly downing Tamworth 4-2 before smashing Grafton 4-nil.
Their winning streak continued on Sunday, as they downed both Metro South West (3-nil) and Central Coast (5-nil).
Illawarra South Coast, which was captained by Callum Mackay, booked their spot in the big dance by thumping Lithgow 10-2 on Monday morning - to set up a showdown with Newcastle.
In the decider, goals from Kieran Govers, Daine Richard and Alex Mackay set up Illawarra South Coast's 3-nil win, to seal their state championship.
In the women's side of the draw, Illawarra South Coast, headlined by Gerringong pair Mackenize Ford and Neive Campbell, finished second after losing their grand final 2-nil to North West Sydney.
