Emergency services were called to the southern end of the bridge around 9.45pm after reports a person had been hit by a Toyota HiAce van.
The man suffered compound fractures to both legs as well as possible pelvis and head injuries.
He was treated at the scene by NSW Ambulance paramedics and was then transported by ambulance to the Berry Sporting Complex where they met the Toll NSW Ambulance Rescue helicopter and was airlifted to St George Hospital.
The driver of the vehicle suffered shock.
Police said investigations into the incident were continuing but confirmed the man was on the road for an unknown reason when he was struck by the van.
The two most western lanes of the northbound bridge were closed during the incident, but Princes Highway traffic was still able to travel north on the third lane.
