Police and emergency services were called to Cabramatta Road, near the intersection of Bauer Road, in Cabramatta West, about 1.15pm.
Initial investigations suggest a Honda Jazz and a Citroen Xsara collided head-on, killing a female passenger, believed to have been in her 70s.
NSW Ambulance paramedics treated four other patients at the scene. A man believed to be in his 70s, a woman thought to be in her 50s and a man and woman both believed to be in their 20s were all taken to Liverpool Hospital in critical conditions.
Investigations into the crash are still under way, with a crime scene established at the crash site.
