Police say the teen was riding along Leabons Lane in Seven Hills on Thursday afternoon when Highway Patrol officers tried to stop him.
He allegedly failed to stop and drove away, before crashing into a grey Lexus sedan.
Paramedics were called to the scene at 4.30pm and found the teen had suffered a fracture to the leg and head injuries.
Leabons Lane was closed while the boy was treated by paramedics and a Careflight doctor.
He was taken by ambulance to Westmead Hospital in a serious condition.
The driver of the Lexus, who was not injured, was taken to hospital for mandatory testing.
Vision from the scene showed a red trail bike, which appeared to be missing its front wheel, lying damaged near the gutter.
Numerous Highway Patrol cars and unmarked police cars filled the road as the crash scene was taped off.
The road remained closed for several hours after the crash, as police investigated how it unfolded.
Westmead Hospital said the boy remained in a serious condition on Thursday night.
