Police said two adults and two children were travelling north on the Princes Highway in a black Ford Falcon XR6 sedan about 8.30am, when it left the road and crashed into two trees.
Witnesses contacted emergency services and rendered medical assistance until help arrived.
Three occupants were trapped in the vehicle, before being released by NSW Ambulance Rescue.
The driver, a 32-year-old woman, and front-seat passenger, a 32-year-old man, sustained serious injuries and were treated by NSW Ambulance Paramedics, before being airlifted to Liverpool Hospital.
Both remain in a critical but stable condition.
The two back-seat passengers, a seven-year-old girl and nine-year-old girl were also seriously injured and airlifted to Sydney Children’s Hospital at Randwick.
The seven-year-old remains in a critical condition.
The nine-year-old is conscious and in a stable condition but remains in the intensive care unit.
Specialist officers from the Crash Investigation Unit conducted an examination of the scene and the vehicle was seized for forensic analysis to determine if mechanical failure may have led to the accident.
The driver will also undergo mandatory drug and alcohol testing.
Officers from the Crash Investigation Unit are continuing their investigation into the accident and are asking for any witnesses who have not yet spoken with police to come forward.
