Police said a Toyota utility was travelling east on Turpentine Road about 4.55pm, when it lost control, rolled a number of times before colliding with a large tree.
The accident happened about 4.5km from the intersection with the Princes Highway on a gravel section of the road before converting to tar.
The 85-year-old male driver and 72-year-old female passenger were both trapped in the vehicle and had to be released by NSW Ambulance Rescue.
The driver sustained serious head and spinal injuries and the passenger suffered serious leg injuries.
Both were airlifted to St George Hospital where they are in a stable condition.
Southern Region Crash Investigation Unit attended the scene and conducted investigations into the cause of the accident.
The accident comes just days after the announcement of extra federal black spot funding to seal a further 9.6km stretch of the accident prone road.
Shoalhaven City Council received a further $824,000 under the 2013-14 Black Spot Program to seal a section of the road.
Together with a previous $2 million funding commitment for sealing, it will allow between 6.6 and 9.6km to be tarred west of the Princes Highway.
